- Provided companionship to a dementia client, listened to him and encouraged him to do light physical activities. Ensured and maintained safe environment. -Provide healthy meals and light housekeeping. - Check vital signs and perform head to toe physical assessment. - Making sure the environment is safe such as the bed rails are up and bed are at the lowest level, decluttering the space, keep the floor dry and have adequate light. -Change simple dressing and check blood glucose level. -Help with ambulation and mobility. -Assist with meals, morning care such as toileting, brushing teeth, bathing and changing clothes. -Maintain documentation such as create care plans, record vital signs and mark progress.
