Duties of our Care Manager: • Build and nurture relationships with clients, field staff and the community • Collaborate to create and deliver a Care Plan • Lead or assist with the integration of client satisfaction initiatives; ensure that all referred client concerns and complaints are satisfactorily addressed and documented in a timely manner. • Assume responsibility for specific Branch Office operations in the absence of the Area Director • Provide in-person and virtual support for clients and caregivers as necessary on a 24/7 basis • Complete other tasks as requested.
