Following the care plan, observing and reporting any substantial findings and/or changes in patient/resident/client’s behaviour to the appropriate registered healthcare team member. Assist nurses and physical therapy staff with the personal care of patients and physical therapy goals. Assisting and providing care to the patients who are associated with a combination of multiple chronic conditions, mental health issues, medication-related problems, and social vulnerability. Provide bedside and personal care to clients and help them move, bathe, groom, and get dressed and undressed. Provide bed bath, pericare and groom to patients with ventilator as well as measuring urine, cleaning and emptying Foley catheters. Support patients in transferring, moving, adjusting, and getting out of bed using an assistive device such as a Hoyer lift. Always ensure the health, safety, welfare, and independence of clients Helping clients with leisure activities. Assisting and satisfying the social and emotional needs of a client. Assistance with toileting, bathing, dressing, and preparing clothes to wear based on her/his choice. Working with elderly patients and providing one-on-one care such as assisting with personal care, meal preparation, and light housekeeping. Documenting and recording daily care reports which may include resident incident reports, restriction monitoring and medication administration. Adhering to the Occupational Health and Safety Regulations set by the province or territory.
