Succor Clinic 1 Support client to attend appointments such as medical and family meetings. 2 Client support with daily living skills, community integration, recreation, residential living, and individual goal achievement 3 Assess and assist clients in managing symptoms through redirection, medication management, education, communication through verbal and written reporting 4 Monitor medication compliance, provide reminders, assess and document compliance, report concerns to Case Manager(s) 5 Assess and support client’s personal care routine, nutrition and meal preparation, laundry and basic household maintenance 6 Direct interaction and rapport building with clients in a variety of situations 7 Crisis management and interacting with clients or community resources such as EMS in a fast paced or stressful situation. 8 Housecleaning
